Service Description: Represents a team of local health professionals, organizations and community members working to create a network where citizens will have access to the right care, right team and right care setting when they need it; to experience seamless transition throughout their care journey in a system that is understandable, digitally-enabled and collaborative

Offers a new model of organizing and delivering health care for residents of the Kenora District that better connects patients and providers in their communities to improve patient outcomes
  • Currently comprised of Indigenous, municipal and health care members and other collaborating organizations and individuals that represent health and social services across the Kenora area
  • Health care providers (including hospitals, doctors, long-term care and retirement homes, acute care, children's services, home and community care providers, Indigenous care and digital health group) and social service providers work as one coordinated team
  • All Nations Health Partners OHT members collaborate with patients, clients, family members, caregivers, individuals with lived and living experience and other citizens to co-design the best possible care for the region
  • Serves as the smallest OHT and the only team with full Indigenous and community partners
