Service Description: Provides programs and services to meet the needs of individuals of all ages with an intellectual and/or developmental disability

Provides opportunities for individuals to become an integral part of a family within the community
  • Recruits and screens families willing to welcome an adult into their home as part of their family, and who will promote and provide a safe and secure place to live, a high quality of life, and support and community inclusion
  • Matches the individual to the host family with consideration to mutual interest and living preferences
  • Maintains ongoing contact, home visits, case management and supervision after placement

Application: Funding support for Host Family arrangements approved through Developmental Services Ontario (DSO) (1-855-376-6673)

  • Intake assessment required

  • Signed Memorandum of Understanding required between consumer, host family and Association
Eligibility / Target Population:
  • Individuals 18 years of age and older with an intellectual and/or developmental disability who would benefit from living in a family setting family and

  • Families willing to envision a lifetime commitment to sharing their home with an adult requiring support

  • Family must be willing to allow external monitoring and advocacy for the dependent consumer and

  • Agree to an in-depth annual review of the arrangement
