Service Description:
|
Provides programs and services to meet the needs of individuals of all ages with an intellectual and/or developmental disability
Enriched Supported Independent Living (ESIL)- Offers support services in an individual's own home for those who do not require 24 hour support
- Supports vary with each person and can include budgeting, attending medical appointments, daily meal preparation, housekeeping and developing relationships
Intensive Supported Independent Living (ISIL)- Uses the "Housing First" model for individuals with significant barriers to housing
- Provides supports for individuals who have difficulty securing and maintaining housing
Outreach Supported Independent Living (OSIL)- Provides case management for individuals with complex needs including an intellectual disability, serious mental illness, and addiction
- Supports individuals with the transition from youth to adulthood

- Supports are available from 2-24 hours per day, 7 days per week
