Service Description:

Provides programs and services to meet the needs of individuals of all ages with an intellectual and/or developmental disability
Offers direct assistance for adults with a developmental disability who are living on their ownAssists adults with an intellectual disability to acquire skills for daily living and to access any community supports and servicesProvides the following services:- Case management
- Assistance to coordinate community resources
- Development of service plans with other service providers
- Emotional support during stressful life changes
- Information about other available resources, supports and services
