Service Description: Develops and implements policies, programs and a service system that helps improve outcomes for children, youth, families and individuals who need support, and advance the interests of women across Ontario

Provides funding to assist families who are caring for a child with a developmental and/or physical disability for:
  • primary caregiver relief and related supports
  • activities of personal growth or development for the child, including camps and recreational activities, like swimming and music classes, which can help the child learn new skills and abilities, such as improving their communication skills and becoming more independent
The amount of funding received depends on:
  • the type and amount of service the child needs
  • what other help is available in the community
  • what kind of support they already receive

Application:
  • Apply online here or download and complete application on the website

  • Attach medical statement or psychological assessment to the application describing the disability, why services are needed and how much it will cost

  • Mail the completed application and medical assessment to: Regional Office, 199 Larch St, Suite 601, Sudbury ON P3E 5P9

Eligibility / Target Population:
  • Children 17 years of age and younger

  • Must have a developmental and/or physical disability

  • Individuals must live in Ontario with their families, or does not live at home but is not being helped by other residential services

  • Must need help with daily activities and require more support than families can usually provide
