Service Description: Family Health Team operates a health clinic in two communities and includes: Nurse Practitioner, Registered Practical Nurses, Registered Social Worker, Registered Dietitian, Administration and other professionals. Family Health Teams allow physicians and other allied health providers to work as a team to focus on keeping patients healthy.

Provides education and services for the prevention, management and treatment of diabetes
  • Provides case management and screening services
  • Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes
  • Advises on developing and implementing an approach to care and management of diabetes
  • Communicates patients' concerns to family physician
  • Assists individuals in finding appropriate services
  • Provides referrals to other health care professionals

Fees: Most services covered by OHIP
Application:
  • Requires booked appointment

  • No referral required for most services

  • New patients may roster with a Physician by completing enrollment form available at office
Eligibility / Target Population:
  • Patients of the JE Stokes Medical Centre and the Aguasabon Medical Clinic living with diabetes or at-risk of developing diabetes

  • Must have a valid Ontario Health Insurance Plan (OHIP) card or other provincial health coverage
