Service Description: A non-profit organization that allows individuals with a disability as well as seniors to remain active and independent within their community

  • Provides attendant and support services to persons with physical disabilities and seniors living in their own homes or in supportive housing
  • Focus is to empower individuals to live safely and independently in the community
  • Offers services based completely on needs of individual
  • Programs and services include the following:

    • Attendant Services
    • Fee for Service
    • Community Outreach, Support Services

    • Open to referrals from any source

    • Requires completed application available at office or on website

    • Application is reviewed and approved by Admissions Selection Committee

    • Interview will be arranged with Program Coordinator
    Eligibility / Target Population:
    • Individuals 16 years of age and older with a disability related to visual, hearing, mobility impairment, acquired brain injury as well as conditions related to aging and/or medical illnesses

    • Individuals 60 years of age and older who require access to 24 hour services
