Service Description:

The Red Lake Family Health Team includes: Family Physicians, a Nurse Practitioner, Registered Nurses, Dietitian, Social Workers and Administration- Family Health Teams allow physicians and other
allied health providers to work as a team to focus on keeping patients healthyThe vision calls for physicians, nurse practitioners and other members of the team to practice together in a positive working environment, sharing and benefiting from the complementary knowledge and skills of their colleagues
Operates a primary care medical clinic consisting of family doctors, nurse practitioner and registered nursesProvides access to other health care professionals such as social workers, dietitian, and diabetes educators within and outside of the Medical AssociatesCoordinates documentation if patient receives care from a member of the teamProvides a walk-in clinic for urgent careOffers primary health care including but not limited to:- Chronic disease management
- Disease prevention
- Health promotion
- Mental health counselling
- Nutritional counselling
- Palliative care
- Rehabilitation
