Service Description: The Canadian Red Cross Society is part of the largest humanitarian network in the world, the International Red Cross and Red Crescent Movement
This network includes the International Committee of
the Red Cross (ICRC), the International Federation of Red Cross and Red Crescent Societies (Federation) and 192 National Red Cross and Red Crescent Societies dedicated to improving the situation of the most vulnerable throughout the world

Offers assistance to patients who are transitioning from hospital to home
Services may include:
  • Assistance with transportation
  • Home configuration
  • Shopping
  • Finding other community support services to help

Application: Potential clients are identified by the hospital or via Ontario Health atHome - North West Area and bookings must be received a minimum of 24 hours in advance
Eligibility / Target Population: Individuals 55 years and older who:

  • are able to direct own care or caregiver

  • are medically stable

  • are able to be left unattended

  • are mobile, ambulatory (able to self-transfer), and not need the assistance of more than one person to manage stairs

  • pose no threat to the health and safety of self or workers AND

  • do not live in a long-term care home or Assisted Living where 24-hour care is provided
