Operates as a group of family physicians who provide comprehensive health care
Operates as a group of family physicians who provide comprehensive health care- Offers timely access to high quality primary care
- Provides access to other health care professionals within and outside of the Family Health Network
- Coordinates documentation if patient receives care from a member of the team
- Provides a walk-in clinic for urgent care
Offers primary health care including but not limited to:- Chronic disease management
- Health promotion
- Disease prevention
- Nutritional counselling
- Mental health counselling
