Service Description:
|
Multi-service agency providing client-centred programs and services for adults with a developmental disability
Provides support services for recipients of Passport funds which can be used for but not limited to the following services: - Management of all the paper work in applying for Passport funds
- Administration of the funds for desired supports and services; can cover transportation costs such as taxis/buses
- Person centred planning
- Community participation such as bowling, swimming, concerts, sports events etc, as directed by client
- Out of town trips for shopping, concerts or visiting family/friends
- Support in activities within client's own home such as cooking and crafts
- Caregiver respite for family and caregivers
