Service Description:

Provides a variety of rehabilitation services, mental health and addiction services, and long-term care services
A goal-oriented outpatient neurology rehabilitation service for clients with neurological conditions- Helps clients maximize independence and their functional potential
- Assists clients to independently access community resources and maintain their ability to live at home
- Provides the option of having individual or group sessions
- Offers a team of healthcare providers, including: Occupational Therapist, Physiotherapist, Psychological Associate, Rehabilitation Assistant, Social Worker, Speech-Language Pathologist, and Therapeutic Recreation

Self-referral or referral by Physician or Nurse Practitioner required

Eligibility / Target Population:
|
- Individuals 15 years of age and older and a resident of NW Ontario
- Have a Neurological diagnosis (stroke, acquired brain injury, MS, ALS, Parkinson's Disease, or spinal cord injury
- Require professional intervention
- Actively participate in therapy for a minimum of one hour per visit
- Be independent with toileting or have caregiver assistance
