Service Description: Coordinates the delivery of health care services to the Anishinabe people across the Sioux Lookout region

Offers a mobile Indigenous Interdisciplinary Primary Care Team, providing services to the Sioux Lookout First Nation Health Authority communities as well as the Municipality of Sioux Lookout
Teams will travel to communities on a regular basis to provide collaborative care in the following areas:
  • Community Health Workers
  • Nursing (Registered Nurses, Registered Practical Nurses, Nurse Practitioners)
  • Nutritional Consultations and Food Care
  • Occupational Therapy
  • Pharmacy Consultation
  • Physiotherapy and Kinesiology
  • Pharmacist
  • Registered Dietician
  • Speech Therapy
  • Telemedicine Services
  • Traditional Healing
