Service Description: The Dryden Area Family Health Team includes: Nurse Practitioners, Registered Nurses, Registered Practical Nurses, Registered Dietitians, Occupational Therapist, Registered Pyschotherapist, Patient Care Navigator and Administration. Family Health Teams allow physicians and other allied health providers to work as a team to focus on keeping patients healthy.

Memory education and exercise
  • Provides internal and external memory strategies
  • Offered twice a year and each class runs 60-90 minutes
  • Provided by an Occupational Therapist

Fees: None
Application: Self-referral or referral by a Physician, Nurse Practitioner, or other healthcare provider
Eligibility / Target Population: Individuals at the Dryden Family Health Team with mild cognitive impairment/normal age-related changes in memory
