Service Description: The Dryden Area Family Health Team includes: Nurse Practitioners, Registered Nurses, Registered Practical Nurses, Registered Dietitians, Occupational Therapist, Registered Pyschotherapist, Patient Care Navigator and Administration
  • Family Health Teams allow physicians and other allied health providers to work as a team to focus on keeping patients healthy
  • The vision calls for physicians, nurse practitioners and other members of the team to practice together in a positive working environment, sharing and benefiting from the complementary knowledge and skills of their colleagues


Offers education and exercise classes twice a year for people at risk of falling or with mobility issues
  • Aims to improve strength and balance to prevent injury and falls
  • Classes are run by a Physiotherapist and are 60-90 minutes in length for 6 weeks

Fees: None
Application: Self referral, physician or healthcare provider through Family Health Team
Eligibility / Target Population:
  • People at fall risk

  • People with mobility issues
