Service Description: Provides a variety of rehabilitation services, mental health and addiction services, and long-term care services

Program that helps individuals manage their arthritis
  • Provides an inter-professional approach with active participation based on client goals
  • Offers individualized assessment, intensive treatment and self-management education to help clients manage their disease process
  • Self-management programs include:
    • Living with Fibromyalgia (runs 2 times a year)
    • GLA:D, 6-week exercise and education program for knee and hip osteoarthritis
Offers the following:
  • Educational sessions
  • Hand/foot class
  • Group/individual exercise
  • Pool exercises
  • Splinting/orthotics

Fees: None
Application: Physician or Nurse Practitioner referral required
Eligibility / Target Population: Primary medical diagnosis of arthritis with priority given to the following:

  • New onset inflammatory and non-inflammatory arthritis

  • Individuals having difficulties managing functional abilities
