- Provides access/referrals to other health care professionals within and outside of the Medical Associates and Family Health Team
- Coordinates documentation if patient receives care from a member of the team
- Provides a Walk-in clinic
Offers primary health care including but not limited to:- Chronic disease management
- Health promotion
- Disease prevention

Eligibility / Target Population:
|
- Must be rostered with a Physician of the Family Health Clinic to book an appointment
- Not accepting new patients but will notify public via website with any changes
