- Once application is approved, applicants are required to provide a photograph (headshot, with face visible on a clean background with no hat or sunglasses)

- Must be an adult, child, or youth with a disability that restricts from participating in or performing activities
- Disability cannot be eliminated by the use of a technical aid such as eyeglasses
Temporary PAL cards available for individuals pre/post surgery or recovering from an accident
