Service Description:
|
Provides culturally appropriate health and social services to member First Nation communities
Assists individuals by providing support services to enable them to remain in their own homes instead of long term care or hospital settings- Offers services of Home Makers for assistance with indoor chores and Home Support staff for assistance with outdoor work
- Provides services for the Elderly and disabled as well as prenatal, postnatal and post surgical care patients including routine household activities such as light housekeeping, laundry, shopping, meal preparation and personal care
- Provides assistance with heavier outdoor work such as shovelling, yardwork and firewood
- Acts as respite for caregivers

Eligibility / Target Population:

- Members of the participating First Nation communities who are 60 years of age and older and require home-making/home support assistance or
- are living with a disability, or are prenatal or postnatal, or require post surgical care
