Service Description:	Provides culturally appropriate health and social services to member First Nation communities Provides a confidential electronic Registry for member First Nations to collect and store individual and community information Developed by Kenora Chiefs Advisory and maintained by Health Directors/Clerks of individual communities Participation is voluntary for all community members; consent form provided for those wishing to be removed from Registry Contains information such as name, birth date, gender, address, Indian status and contact information; may also contain traditional name and/or clan membership
