Service Description:

The Red Lake Family Health Team includes: Family Physicians, a Nurse Practitioner, Registered Nurses, Dietitian, Social Workers and Administration- Family Health Teams allow physicians and other
allied health providers to work as a team to focus on keeping patients healthyThe vision calls for physicians, nurse practitioners and other members of the team to practice together in a positive working environment, sharing and benefiting from the complementary knowledge and skills of their colleagues
Provides additional health services to community members that can include:- Assistance with filling out various forms (Ontario Works, Employment Insurance etc)
- Information and counselling about smoking cessation
- Information regarding Power of Attorney forms
- Chronic disease self-management groups
- Immunizations
- Cancer Screening
- Cholesterol Management workshops
- Blood Pressure (BP) Clinics
- Walking Group
Doctor-referred programs available include:- Senior's Health Day
- Cardiovascular Risk Assessments
- Spirometry testing and Asthma/COPD counselling
- Ankle Brachial Index (ABI) Testing
