Service Description: The Sunset Country Family Health Team includes: Physicians, Nurse Practitioners, Registered Nurses, Registered Practical Nurses, an Occupational Therapist, Dietitians, Pharmacist, Chiropodist, Social Worker, Consulting Psychiatrist and Administration
  • Family Health Teams allow physicians and other allied health providers to work as a team to focus on keeping patients healthy
  • The vision calls for physicians, nurse practitioners and other members of the team to practice together in a positive working environment, sharing and benefiting from the complementary knowledge and skills of their colleagues


  • Assists patients living with asthma or Chronic Obstructive Pulmonary Disease (COPD) to better manage their disease process
  • Conducts spirometry assessments to determine lung function
  • Provides education and support to help set goals for individual's care
  • Offers smoking cessation counselling if needed
  • Arranges follow-up visits as needed
  • Provides referrals to other programs and services
