Supports for individuals living with pre-diabetes or diabetes- Provides Certified Diabetes Educators to help individuals manage their diabetes
- Assesses, monitors, and assists individuals with diabetes management and to attend to any diabetes complications they may have
- Offers education and self-management support to fit an individual's lifestyle
Offers the following interventions:- Complication/Co-morbid condition management
- Insulin starts and adjustments
- Healthy Lifestyle
- Meal Planning
- Nutritional assessments
- Pregnancy management
- Type 1 and Type 2 management

Eligibility / Target Population:
|
Individuals with pre-diabetes, type 1 and type 2 diabetes, gestational and pre-gestational diabetes, or are at risk for the development of diabetes
