Service Description:
|
Provides culturally appropriate health programs and social services to the member First Nation communities
Provides off-site supervision and support to member communities in their implementation of the Community Wellness Worker (CWW) program- Promotes healthy lifestyles through culturally based programs and healing
- Addresses and aims to reduce family violence
Program delivered at the community level consists of the following services:- Provides assistance for individuals to identify areas of concern and to develop action plans to address concerns
- Liaises and networks with related health and social agencies and provides referrals
- Provides crisis intervention education and workshops
- Offers illness prevention and wellness planning, information and education
- Provides community outreach education to promote cultural awareness
