Service Description:
|
Provides culturally appropriate health programs and social services to the member First Nation communities
Support for people living with pre-diabetes or diabetes- Offers education and self-management support to fit an individual's lifestyle
- Provides access to a Registered Nurse and Registered Dietitian
Provides outreach services to First Nation clients living with diabetes in the following areas:- Education
- Monitoring and screening
- Diabetes compliance, prevention and intervention
- Networking and liaison with the Dryden Regional Health Centre
- Referral and liaison to other programs as required

First Nation individuals living in the communities within the Dryden Tribal Area with pre-diabetes, type 1 and type 2 diabetes
