Service Description: Responsible for the delivery of provincially mandated services on behalf of the citizens of the District of Thunder Bay

Provides various health benefits to individuals and families with high health costs
Benefits are determined based on need and may include:
  • basic dental care
  • diabetic supplies
  • eyeglasses
  • hearing aids
  • prescription drugs
  • surgical supplies and dressings
  • transportation for medical appointments

Fees: None
Application: Call for information
Eligibility / Target Population: Individuals and families who:

  • are between 16 and 65 years of age

  • have high prescription drug costs or other health related costs

  • were eligible for Ontario Works in the previous months, or

  • are no longer eligible for Ontario Works because they received a payment for loss of income under the 1986-1990 Hepatitis C Settlement Agreement
