Service Description: A collaboration between four healthcare agencies that work together to make it easier for individuals to access mental health or support services

  • Serves as a single access point for individuals searching for outpatient mental health services, supportive housing, case management, diagnostic assessment/medication review, and chronic pain management services
  • Works as a partnership between four community health organizations including Alpha Court, Canadian Mental Health Association, St Joseph's Care Group, and the Thunder Bay Regional Health Sciences Centre for the services they offer
  • Provides individuals with a simplified outpatient referral form to access programs or services

  • Fees: None
    Application:
    • Referrals can be made by an individual, a health care professional, or on behalf of someone else

    • The referral form is available on the website

    • The form can be submitted electronically or can be downloaded, printed, and submitted via fax or mail


    Note: Some services require consent of a primary healthcare provider, who will then be contacted to confirm that consent has been given
