Service Description: Operates a self government and is responsible for the day to day operations of the Rainy River First Nation

Provides education and services for the prevention, management and treatment of diabetes
  • Provides case management and screening services
  • Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes
  • Advises on developing and implementing an approach to care and management of diabetes
  • Assists individuals in finding appropriate services
  • Provides referrals to other health care professionals
Provides programs and services including:
  • Annual poker walk for all ages with grand prizes
  • Basic foot care clinics
  • Breakfast and lunch programs
  • Community kitchen
  • Fitness and nutrition programs
  • Glucose monitoring
  • Pre-natal education
  • School diabetes program
  • Support groups

Application: Appointments recommended
Eligibility / Target Population: Members of Rainy River First Nations living with diabetes or at-risk
