Service Description:
|
Provides supportive housing and community support services for seniors and community members with disabilities
Non-emergency transportation service to local medical appointments and regular activities
- Partners with Dryden Native Friendship Centre to provide transportation services
- Provides transportation to medical appointments, shopping, and social activities
- Works by providing up to 6 discount taxi vouchers per month

- Initial assessment required
- Requires completion of a photo identification card (valid for 1 year); done during initial visit
|

Eligibility / Target Population:
|
Must be 60 years of age or older and meet one of the following criteria:
- The client is in receipt of or eligible to receive other long term care community services
- The client is isolated and requires social interaction or stimulation
Transportation
Client must meet one of the following criteria:
- Unable to access public transportation because of short or long term physical limitations
- Requires escort during the trip because of frailty or cognitive impairment
- Specialized transit is not available or the individual does not meet the eligibility criteria for Handi-Transit
- Public transportation is not available and friends or relatives are not available or able to provide transportation
