Service Description: Operates a self government and is responsible for the day to day operations of the Whitesand First Nation
  • Tribal Council: Independent First Nations Alliance (IFNA)
  • Robinson-Superior Treaty


Supports for individuals living with prediabetes or diabetes
Provides education and services for the prevention, management and treatment of diabetes
  • Provides case management and screening services in cooperation with the Diabetes Team from the St Joseph's Care Group in Thunder Bay
  • Offers basic foot care services
  • Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes
  • Advises on developing and implementing an approach to care and management of diabetes
  • Assists individuals in finding appropriate services
  • Provides referrals to other health care professionals

Eligibility / Target Population: Members of Whitesand First Nation living with prediabetes, Type 1 diabetes, Type 2 diabetes, or who are at-risk
