Service Description:

Operates a self government and is responsible for the day to day operations of the Whitesand First Nation- Tribal Council: Independent First Nations Alliance (IFNA)

Supports for individuals living with prediabetes or diabetes
Provides education and services for the prevention, management and treatment of diabetes- Provides case management and screening services in cooperation with the Diabetes Team from the St Joseph's Care Group in Thunder Bay
- Offers basic foot care services
- Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes
- Advises on developing and implementing an approach to care and management of diabetes
- Assists individuals in finding appropriate services
- Provides referrals to other health care professionals
