Service Description: Operates a self government and is responsible for the day to day operations of the Pikangikum First Nation

Provides education and services for the prevention, management and treatment of diabetes
  • Provides case management and screening services
  • Offers basic foot care
  • Offers walking programs and exercise programs tailored to meet the needs of the individual
  • Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes
  • Advises on developing and implementing an approach to care and management of diabetes
  • Assists individuals in finding appropriate services
  • Provides referrals to other health care professionals

  • Appointments highly recommended

  • Appointments required for basic foot care sessions
Eligibility / Target Population: Members of Pikangikum First Nation living with diabetes or at-risk
