Service Description:

Promotes living beyond mental illness and addiction through the provision of individualized, recovery-oriented, community-based services that are developed in partnership with consumers
Provides case management services to adults living with a mental illness and a developmental disabilityPromotes recovery and independence while providing appropriate community based supportProvides the following services:- Assistance in accessing mental health and/or psychiatric services
- Help to integrate into the community, advocating with informal and formal community resources
- Referrals to community agencies, where appropriate
- Assistance in accessing income support such as Ontario Works and Ontario Disability Support Program
- Housing rent supplement
- Assistance in obtaining and maintaining housing in the community
