Service Description: Operates a self government and is responsible for the day to day operations of the Couchiching First Nation

Delivers medical transportation services and assistance with processing Non-Insured Health benefits claims
  • Provides transportation from/to Couchiching Reserve to any medical appointments (doctor, dentist, optometrist etc) in the town of Fort Frances
  • Submits medical appointment travel, mileage, meal and accommodation requests on behalf of individual for out-of-town medical appointments through the Non-Insured Health benefits program
  • Provides assistance with making arrangements for any medical appointments out of town such as booking appointments, hotels, and completing applications for mileage compensation

Fees: None
Application: Local Medical Transportation

  • Requires written proof of medical appointment

  • Requires 24 hour advance booking; exceptions made for emergencies

  • Advance cancellation recommended


Out-of-Town Medical Assistance

  • Must make arrangements for out-of-town travel at least 2 weeks prior to date of appointment/travel to allow time to process Prior Approval package with Health Canada

  • Must have Confirmation of Appointment (COA-blue slip) stamped/signed and return to Health Centre after appointment
