Service Description: Addresses the needs of Métis peoples by providing information and administering programs and services available through the Métis Nation of Ontario

Operates as a program under the Healing and Wellness Branch
  • Provides information, advocacy and support to assist individuals to remain in their homes as long as possible
  • Acts as respite for informal caregivers such as extended family
Community Support Services offered include:
  • Caregiver support
  • Case management
  • Friendly visiting
  • Service arrangement and coordination
  • Transportation for medical appointments within Fort Frances and/or assistance with medical appointment arrangements
  • Visiting for social and safety reasons, either in person or on the phone

Fees: None
Application:
  • Intake assessment required

  • Transportation arrangements must be booked 24 hours in advance; cancellation as soon as possible appreciated
Eligibility / Target Population:
  • Focus is on frail, vulnerable and at-risk elderly adults and individuals trying to overcome barriers in accessing care such as low income, geographic isolation, language or disability issues or

  • Self-identified Métis persons of any age who require care in order to remain in their homes
