Service Description:	Serves as a paediatric outpatient health facility and provides a variety of programs for children and youth with physical disabilities Helps individuals lead more independent lives by assessing mobility and positioning needs Prescribes mobility and/or positioning devices

Application:	Referrals accepted from medical, social, and educational professionals Referrals can be made through the online referral form, found here or call the centre for assistance
Eligibility / Target Population:	Infants, children and youth 21 years of age and younger who have a physical challenge affecting function, mobility, positioning, posture and/or skin integrity
