Service Description:

Provides a multi-site, multi-function health care system serving the residents of the Rainy River District
Provides case management and community-based support services for individuals experiencing psychiatric symptomsAdvocates for the improvement of services delivered to individuals with psychiatric issuesCan also provide:- Advocacy and referrals
- Assessment and monitoring
- Assistance to access housing, education, employment and health services
- Crisis intervention
- Life skills training
- Medication management and monitoring
- Rehabilitation
- Service coordination
- Supportive counselling

Eligibility / Target Population:

- Individuals 18 years of age and older experiencing psychiatric symptoms
- Must voluntarily agree to participate in the program
