Service Description:
|
Operates a self government and is responsible for the day to day operations of the Cat Lake First Nation- Tribal Council: Windigo First Nations Council

Organization: Nishnawbe-Aski Nation (NAN)Treaty #9
Responsible for processing Non-Insured Health Benefits Service claims and coordinating medical transportation for community members- Provides transportation within the community for individuals accessing the Nursing Station
- Submits medical appointment travel, mileage, meal and accommodation requests on behalf of individual
- Provides assistance with completing prior approval applications for long distance medical appointments
- Offers assistance with arrangements for accommodation and travel details for individuals attending medical appointments at Sioux Lookout Meno Ya Win Health Centre

Application:

- Must make arrangements for out-of-town travel at least 2 weeks prior to date of appointment/travel
- Requires written proof of medical appointment
- Must have Confirmation of Appointment (COA) stamped/signed and return to Health Centre after appointment
- Advance cancellation recommended

Eligibility / Target Population:
|
Members of Cat Lake First Nation who require assistance with Non-insured Health Benefits (NIHB) arrangements and forms, and assistance with medical transportation due to age, illness or disability
