Eligibility / Target Population:
|
Individuals 18 years of age and older with mental illness and meet any of the following:
- Are receiving case management services and are appropriately supported by a community-based service
- Are on Ontario Works, Ontario Disability Support Program with a rental agreement that exceeds shelter allowance
- Have a mental illness
- Are at risk of being charged or have been charged with an offence that is considered low risk
- Have a problematic issue with substance use
