Service Description:
|
Provides programs and services to meet the needs of individuals with an intellectual and/or developmental disability
Offers supports to individuals with developmental disabilities living in their own apartments within a supervised setting- Supports can vary from a few hours to 24 hours per day, depending on the individual's needs
- Aims to meet all aspects of the individuals needs, including but not limited to personal hygiene, cooking, cleaning, socialization, financial management and health
- Provides one to one personal support within the home or community
- Teaches and/or maintains daily living skills, problem solving skills
- Determines the degrees and types of supports required by the needs of the individual receiving supports and their families
