Delivers medical transportation services and assistance in making medical appointment arrangements
  • Provides transportation to medical appointments within the community
  • Provides transportation assistance from/to Sachigo Lake First Nation to medical appointments (doctor, dentist, optometrist, specialist) in Sioux Lookout and Thunder Bay
  • Provides assistance with making arrangements for any medical appointments out of town such as booking appointments, hotels, and completing applications for mileage compensation

Application:
  • Requires written proof of medical appointment

  • Requires 24 hour advance booking; exceptions made for emergencies

  • Advance cancellation recommended
Eligibility / Target Population: Members of Sachigo Lake First Nation who require medical transportation assistance due to age or disability
