Service Description: Operates as a community-based organization offering primary health care and health promotion programs in areas within the district

  • Offers a primary mobile health care unit to individuals living in the district
  • Serves individuals who are having difficulty finding health care, or who have barriers and access issues, such as geography, or who may be at a higher risk of poor health
  • Offers a multidisciplinary health care team including registered nurses, nurse practitioners, footcare nurses, community health workers, and a dietitian
  • Diagnoses, treats and refers clients within their scope of practice
  • Treats cuts, common colds, fever, earaches, minor infections, bruises, sprains and skin skin complaints
  • Offers regular foot care by appointment

  • Fees: Most services covered by OHIP

    • To become a patient, drop by during one of the Mobile Clinics and pick up a request form
