Service Description: Provides a range of responsive individual, family and community programs and services for the complete life journey of all Anishinabek people

  • Provides support services for seniors, individuals with critical or long-term illness, and persons with health barriers such as a disability
  • Provides speech and language services support to children attending schools on-reserve
  • Support services include but are not limited to:
    • Adult life enrichment and recreation
    • Basic foot care
    • Homemaking including assistance with meals and cleaning
    • Palliative support
    • Personal care including bathing, washing and dressing
    • Respite care
    • Transportation to medical appointments or errands

    Application:
    • Verbal or written referrals accepted from Dilico employees or First Nations communities receiving Community and Personal Support Services

    • Requires an assessment with a Home and Community Care Manager
    Eligibility / Target Population: Indigenous individuals and families living within the jurisdiction of Dilico and who meet the criteria as determined by Care Managers
