Service Description: Provides a variety of rehabilitation services, mental health and addiction services, and long-term care services

  • Offers an out-patient mental health program with a multidisciplinary approach to treating a variety of mental health issues
  • Utilizes a Decision Guide model to assist client with goal setting, decision making and determination of needs
  • Provides assessment of client's strengths, challenges and barriers to treatment to match client with appropriate services
  • Offers services depending on the needs of client that can include:
    • Counselling services (provided by a social worker, psychological associate, registered psychotherapist or psychologist)
    • Psychiatric Consultation Clinic (including Telepsychiatry)
    • Psychological Assessment Services (diagnosis, psychological functioning and inform treatment plans)
    • Specialized Services (including leisure life skills instructors, occupational therapy, dietitian, social workers, and First Nations spiritual care specialists)
    • Workshops and groups for both treatment and psycho-education

    Application:
    • Referrals are accepted from individual clients, physicians, nurse practitioners, family members and community agencies or

    • Referrals can be made through the Thunder Bay Mental Health Programs Intake Coordinator or a completed Thunder Bay Mental Health Program Referral Form can be submitted through the Access Point Northwest website

    • If the referral is not from a family doctor or nurse practitioner, the doctor or nurse practitioner must indicate that they are aware of and in agreement with the referral since the client will be registered to the program under the family doctor or nurse practitioner
    Eligibility / Target Population: Individuals 16 years of age and older, living in Northwestern Ontario and

    • Are living with mental health issues

    • Have a goal to address their mental health needs

    • Are ready and able to participate in services that are available to meet their mental health needs


    Exclusions

    • When emergency services are required and should be obtained at Thunder Bay Regional Health Science Centre (TBRHSC)

    • When third party assessments are requested, such as independent medico-legal examinations

    • When intensive case management is required

    • When the referral is primarily for marital counseling

    • When clients or their substitute decision makers are not willing to provide consent to treatment
