Service Description:
|
Provides culturally appropriate health programs and social services to the member First Nation communities
Education and services for the prevention, management and treatment of diabetes- Works to increase early detection and to reduce the number of avoidable complications that can arise from uncontrolled diabetes
- Advises on developing and implementing an approach to care and self-management of diabetes
- Promotes increased physical activity and knowledge of healthy eating
- Provides workshops, presentations and education for school children and community members
- Provides services of a chiropodist
- Offers individual, group and school counselling sessions

Eligibility / Target Population:
|
- First Nation individuals living with any type of diabetes
- Individuals with impaired glucose tolerance and/or impaired fasting glucose
- High risk individuals
