Provides a multi-site, multi-function health care system serving the residents of the Rainy River District
Community-based diabetes education centre offering supports for people living with pre-diabetes or diabetes- Offers public presentations regarding type 2 diabetes prevention
- Offers outreach clinics located in Emo and Rainy River
- Provides services through individual education sessions and health promotion activities
- Partners with other organizations to provide the Healthy Food Box program

- Individuals with any type of diabetes including gestational diabetes, type 1 / type 2 pregnancy, impaired glucose tolerance, impaired fasting glucose
- Individuals who are at high risk
