Service Description: Provides community and personal support services to individuals with a developmental disability

  • Supports individuals with a developmental disability who want to live in the community but require additional supports in order to maintain their independence
  • Creates an individualized care plan based on a person's needs and goals
    • Supports include:
    • budgeting
    • cooking
    • cultural and religous activities
    • education
    • employment
    • housing
    • life skills
    • medical needs (dental, vision, mental health)
    • shopping

    Eligibility / Target Population: Adults 18 years of age and older with a developmental disability as well as significant medical or behavioural support needs
