Service Description: Responsible for providing wholistic health programs and services for Indigenous people in the Rainy River District

Supports for people living with pre-diabetes or diabetes
Offers a diabetes management clinic to assist individuals in understanding and managing their illness
  • Provides access to consultations with Registered Dietitian
  • Cooperates with assistance from Registered Nurse
  • Offers basic, advanced and diabetic foot care
  • Offers foot screening clinics
  • Coordinates services of a visiting Chiropodist once per month for three days
  • Provides referrals to other foot care service providers
  • Coordinates group grocery shopping tours to help individuals make healthy food choices and to learn about reading labels
  • Offers presentations and printed material for public education and awareness
Program services are provided through:
  • Individual sessions
  • Group sessions
  • Health promotion activities

Fees: None
Application:
  • Accepts walk-ins or booked appointments; no referral required

  • Booked appointments required for visiting Chiropodist

  • Clients can be seen in office, home or community visits
Eligibility / Target Population: Individuals 18 years of age and older and/or families from any of the 10 First Nation communities in the Fort Frances Tribal area


Serves individuals with the following:

  • Type 1/Type 2 Diabetes

  • Gestational Diabetes

  • Type 1/Type 2 Pregnancy

  • Impaired Glucose Tolerance

  • Impaired Fasting Glucose

  • Pediatric Type 2
