Service Description:
|
Promotes living beyond mental illness and addiction through the provision of individualized, recovery-oriented, community-based services that are developed in partnership with consumers
Provides short-term, immediate support to individuals with mental health and/or addiction issuesOffers support up to 6 months, but will refer individuals to longer-term support services if neededSupports services can include:- Advocacy
- Assistance in obtaining housing
- Assistance with community involvement/participation
- Help with applying for Ontario Works or ODSP
- Education around diagnosis, symptom management
- Referrals to community agencies when appropriate
- Short-term case management

Eligibility / Target Population:
|
Individuals 16 years of age and older who need immediate, short-term support due to a mental health and/or substance use issue
Issues may include:
- Acute mental health issues (suicidal ideation/attempts)
- Homelessness
- Severe addictions
- Recent hospital admissions into the Adult Mental Health program
- Discharge from a corrections facility
- Conflict with the law
