Service Description:
|
Promotes living beyond mental illness and addiction through the provision of individualized, recovery-oriented, community-based services that are developed in partnership with consumers
Delivers sensitive, timely and portable services to individuals who are struggling with a serious mental illness or concurrent disorderProvides a psychosocial model of care to facilitate full and active participation in the recovery process and promote independenceDesigns services to maximize an individual's functioning in the communityOffers case management support servicesProvides the following services:- Assistance in accessing mental health and/or psychiatric services
- Help to integrate into the community advocating with informal and formal community resources
- Referrals to community agencies, where appropriate
- Assistance in accessing income support such as Ontario Works and Ontario Disability Support Program (ODSP)
- Housing rent supplement
- Assistance in obtaining and maintaining housing in the community

- Can self-refer or be referred by a family member, care provider, or agency worker
- Referrals for case management can be made by visiting Northwest Access Point

Individuals 16 years of age and older with a mental illness and/or a concurrent disorder
