Service Description:
|
Provides a variety of rehabilitation services, mental health and addiction services, and long-term care services
Exercise and education for persons living with chronic pain- Provides a 6-week outpatient program that focuses on skills, coping strategies, support, and education
- Provides an inter-professional team to help individuals set goals
Note: A less intensive program is available for clients unable to attend the full 6-week program

Eligibility / Target Population:

- Individuals aged 16 years of age and older with chronic pain that interferes with daily activities
- Individuals who are ready to learn and practice self-management strategies
